The Dorna Titus Future Leaders Scholarship is a College and Trade School scholarship for High School seniors in New York. The scholarship is hosted by Forward Motion Life Success, Inc. (FMLS, Inc.), a 501(c)3 non-profit organization, and is named after the late mother of founder, Dr. Alfred S. Titus, Jr. The scholarship focuses on underserved communities and is designed to help students continue their education after high school.
By joining the challenge you will be doing your part in helping a positive cause, while dedicating a few minutes a day to improving your health. It is my hope that the amount of push-ups you start with on day-1, increases by day-28.
By sponsoring someone who is completing the challenge, you are providing encouragement and support in an area that is important to them... health and goodwill.
This is a great way to stay in shape and keep those New Years resolutions, while supporting a great cause... Education.
How It Works
- You set the amount of Full or Modified Push-ups you'd like to complete daily for the month of February.
- Fill out the registration form joining the challenge, and agreeing to obtain sponsors who will support you in the challenge.
- One registration form per person. Families encouraged.
- Each registrant will be emailed a Push-Up Tracking Sheet to keep track of their daily pushups and a Sponsor Sheet to sign-up sponsors.
- Each sponsor you sign-up pledges a dollar amount, minimum $1, to be donated for each day you reach your push-up goal. All pledges are tax deductible. (28 days + $1 pledge = $28 dollars)
- Fill out the Sponsor Sheet (provided) with the name, email address, and amount the sponsor is pledging, and email a copy by January 31, 2023, to: forwardmotionlifesuccess@gmail.com
- Sponsors will be contacted by email, thanking them for their pledges and notified that they will be informed of the number of days the person they sponsored has completed and the amount and method their pledge can be sent.
- At your discretion, you can take a picture or record a short video of yourself, completing the push-ups daily, and send it to FMLS, Inc. via email, text, WhatsApp, or you can post it to your social media and tag FMLS, Inc.
- For those who give permission, FMLS, Inc. will post a different picture or video of a registrant completing the challenge, daily on social media. All pictures and videos will be destroyed by March 1, 2023. Your privacy and confidentiality will be protected.
- At the end of February, the Push-Up Tracking Sheet should be completed. Email a copy of the completed Push-Up Tracking Sheet to FMLS, Inc. at: forwardmotionlifesuccess@gmail.com
- FMLS, Inc. will notify your sponsors via email, of the number of days you completed the challenge and that the tax-deductible pledge amount can be sent to FMLS, Inc. via PayPal, Zelle, CashApp, etc., by March 15, 2023.
- A receipt will be emailed to each sponsor with the tax deduction ID# for tax purposes.
